Please arrive early on your first appointment so that you can complete your initial paperwork. We recommend that the new Patient forms (see below) are filled-out in advance to minimize wait time. You may complete & submit the new patient form online or print and bring completed form with you on your first appointment.
Items to bring with you on your first appointment at Complete Foot & Ankle Specialty:
- Completed new patient form (if not submitted already). See below details.
- Patient’s current health insurance card and photo ID
- Pharmacy Information – Details of all current prescriptions and/or over-the-counter medication
- Information about patient’s medical and surgical history
- Recent test results, x-rays, or relevant records
